The difference is one is mechanical and the other is not-simply that. One is not more painful than the other if in the right hands no matter what those hand tool folks say. In fact, I have seen messed up permanent makeup with both so you need someone who really knows what they are doing and you like the work they do. Be sure you see lots of examples, interview the artist and be very comfortable with them before you let them touch your face.
In the right hands, permanent makeup is exquisite.
